Breast augmentation surgery involves using implants to fulfill your desire for fuller breasts or to restore breast volume lost after weight reduction or pregnancy. Breast implants have been proven to be safe and does not affect functions of the breast. Nor do they cause or increase the chance of tumors or breast cancer. Even breast feeding can be done normally later. The US FDA has approved silicone gel implants for breast augmentation.Implants are available in various sizes. The scars of surgery are well hidden and the breasts look and feel natural. Fondling of breasts are to be avoided for a month. The incision can be made under the breast, around the nipple or in the armpit. It generally takes two to three hours to complete the entire procedure. The outer surface may be smooth or textured, and implants come in various shapes to meet the individual woman’s needs.

Types of Breast Implants

Implant can be broadly classified as SALINE and SILICONE breast implants depending on its contents.

Saline implants have saline (salt water) solution inside the pliable silicon shell. Saline implants were extensively used prior to USFDA approval for cohesive gel implants. Saline breast implants are very safe, but have shown long term problems of rippling and deflation.

Silicon implants are available in the round or circular (most commonly used) as well as the anatomic or ‘teardrop’ shape. We use only MENTOR brand of implants ( US FDA approved).

Breast Augment Procedure

Performed under general anesthesia, breast enhancement surgery takes about 2 hours to complete. Implants are inserted in any of the following locations-
1. Underneath the gland called sub glandular
2. Beneath the muscle known as sub muscular
3. Both below the muscle (upper half of the implant) and below the gland (lower half of the implant) known as Dual plane.
The best doctor decides to create a specific pocket after taking into consideration the tissue character of breasts and type of implant. Small tubes placed after the surgery for draining off the collecting fluid are removed within 24-48 hours.

Incision for Breast Augmentation

Infra mammary crease approach and the circum areaolar approach are most frequently used for breast augmentation by surgeons all over the world. Since infra mammary crease is a well hidden portion of breast, infra mammary crease approach is the most favored by both doctors and patients. A 5 cm incision is made under the breast in such a way that final scar falls in a natural crease below the breast and fades away in the due course of time. Circum areaolar approach makes use of natural color interface of nipple and breast skin for hiding the scar since incision is made in the lower half circle of the nipple or areola.

Post Breast Enlargement Recovery

It is normal to feel slight discomfort, stretch, swelling and bruising after the surgery. Most women resume light daily activities after 24 hours or so. Desk job can be resumed in 3-4 days time. Diminished sensation over the breast is expected and it goes away in a fortnight or so. Collection of serous fluid or blood, infection, sensory alteration, wrongly positioned implants are some of the complications post breast enlargement surgery. These are rare and can be easily resolved at the hospital. Infection after the breast treatment is less than 1% for which course of antibiotic prescribed by the doctor suffices. If infection persists, then implant are removed and reinserted after a few weeks once infection has been taken care of. In some cases, capsular contracture may occur at a later date wherein body forms a capsule around the implant. This capsule may thicken and disfigure the implant. Two most commonly used approaches to breast augmentation / enlargement are the infra mammary crease approach and the circum areaolar approach. In the infra mammary approach, a fine 5 cm incision is made beneath the breast in such a way that the final scar falls in the crease below the breast. As the final scar falls in a natural crease, it fades well over a period of time. The infra mammary crease is the most hidden portion of the breast and most women prefer to have the access from this area. In the circum areaolar approach, the incision is made in the lower half circle of the areola. The advantage in this approach is that the final scar blends in well in the natural color interface of the areola and breast skin.

The Procedure

The enlargement surgery is performed under general anesthesia. The surgery takes about 2 hours to complete. The space / pocket for the implant is made in any of the following locations- 1. Below the gland (sub glandular), 2. Below the muscle (sub muscular), or 3. Dual plane (where the upper half of the implant is below the muscle and lower half below the gland). The decision to create a particular pocket in each case is made considering the type of implant and tissue characteristics of the breast. Small tubes to drain off the collecting fluid may be placed, which are removed in 24-48 hours.

Post Breast Implant Surgery

Minor discomfort, feeling of stretch, swelling and bruising can be expected. Most patients are able to start normal daily activities starting the next day and return to office work in 2-3 days time. Decreased sensation over the breast can occur and usually recovers in a couple of weeks. Complications are rare following breast augmentation. Early complications include collection of serous fluid or blood, sensory changes, infection, implants malposition. The infection rate in breast implant surgery is less than 1%. Antibiotic treatment results in resolution of the infection in most cases. In cases where the infection has not resolved, the implant will have to be removed and inserted few weeks after complete resolution of the infection. Capsular contracture can occur as a late complication. Our body forms a capsule around the implant. This capsule can thicken and distort the implant in some cases.
